The Citizens walked over defending champions Real Madrid 4-0 at the Etihad in the second leg of their semi-final tie to advance 5-1 on aggregate.
Bernardo Silva led the route with two goals with an Eder Militao own goal and one from Julian Alvarez completing the scoreline.
City, who are also in the runnings for the League and Cup double in England, will face Inter Milan of Italy in the Champions League final on Saturday June 10 in Istanbul, Turkey.
